If you're on a GLP-1, you've probably asked yourself this: does it matter what I eat, or is the medicine doing the heavy lifting anyway? It's a fair question. Here's what the studies found.
A 2025 study of almost 400 adults on GLP-1 medicine found something important. Without any food guidance, most were low on protein and key nutrients. At the same time, they were eating too much fat and salt.
That's not a small detail. It's the difference between weight loss that leaves you strong, and weight loss that leaves you worn down.
What Happens Without a Food Plan {#without-a-plan}

A 2025 study checked what almost 400 adults on GLP-1 or GIP/GLP-1 medicine were actually eating. The results were clear. On average, these patients weren't getting enough protein or key nutrients. At the same time, they were eating too much fat and salt.
The researchers were direct about the risk. Without food guidance, GLP-1 treatment on its own can make nutrient gaps worse, not better.
Here's why. Eating less doesn't mean eating better. It just means eating less. If what's left on the plate is low in protein and high in salt and fat, a smaller amount of food doesn't fix anything. It can make the problem worse, just in a smaller portion.
The muscle risk ties right back to this. Several studies on GLP-1 and body composition point to the same number: protein intake of about 1.2 to 1.6 grams per kilogram of body weight each day, spread across meals, is linked to better muscle protection during treatment. Below that number, the medicine's own appetite-cutting power can work against the body instead of for it.
So, what happens when food gets added to the plan on purpose? That's where the next set of studies gets interesting.
What Changes When Food Is Part of the Plan {#with-a-plan}

Several studies have compared GLP-1 medicine alone to GLP-1 medicine paired with real food and exercise guidance. The same pattern shows up again and again.
1. Adding food and exercise support leads to more total weight loss. Clinical trials show that pairing GLP-1 treatment with structured diet and exercise support leads to more weight loss than the medicine or the lifestyle changes on their own.
2. Real guidance changes what people actually eat. In one study, patients who got dietitian-guided nutrition support alongside their medicine cut added sugar more, and increased protein more, than patients on the medicine with no food guidance at all.
3. Protein plus strength training protects muscle. Research pairing adequate protein with strength training during GLP-1 treatment has shown real ability to protect, and sometimes build, muscle while losing weight. This is the biggest lever a patient can control on their own.
4. Good habits during treatment carry forward after it ends. Research on stopping GLP-1 treatment shows that weight regain is common. But it happens less in people who kept up exercise and good eating habits while on the medicine.
None of this replaces the medicine. And no food reverses or treats a medical condition. But the medicine and the plate do different jobs. Skipping one puts more pressure on the other.

The research keeps landing on the same point. GLP-1 alone and GLP-1 plus real food are not the same outcome. Not in total weight lost. Not in what kind of weight comes off. Not in what happens after treatment ends.
Maybe you're skeptical about adding "one more thing" to a medicine that's already working. That's a fair instinct. But the data shows food isn't extra. It's doing work the medicine was never built to do by itself.
